Error using Simple Newsletter unsubscribe form

Hi, I am in test mode with the newsletter add-on and all seems to be working well except for the Unsubscribe form. I am not sure if it is a problem in the addon or a configuration issue. Using a subscriber email that I successfully subscribed and then sent a test edition of the newsletter, I am trying to use the unsubscribe form to request unsubscribe. The debug console of the browser gets opened and it gives the basic error below: Whoops \ Exception \ ErrorException (E_WARNING) Undefined variable $email I am not sure how to capture the stack trace panel and other details, but if you need those, please advise how best to get that to you. One other note... when I first added the block to my unsubscribe page, I enabled the 'captcha' option, and when the page was published, the captcha image showed, but not the data-entry block for it. It seemed like I was getting a second box to enter the e-mail address in. Thank you for any help with this. Please let me know if I can provide any additional information. -Stephen Enger
